Minor (Child) Media Release

Showing Off Our Superstar Athletes I, the parent or legal guardian of the participating child, grant Phoenix Sports, Inc. (and its affiliates) an irrevocable, royalty-free right and permission to use my child’s likeness in any photographs, videos, audio recordings, or other digital media taken during Phoenix Sports activities. This grant of permission is for any lawful promotional or educational use, including but not limited to: our website, social media, flyers, advertising, and marketing.

Furthermore, I understand and agree that no royalty, fee, or other compensation shall become payable to me or my child by reason of such use.

Privacy & Jersey Names:

We are fiercely protective of your child's privacy. Phoenix Sports will never use your child’s real name or other identifying personal information in our marketing. While we do our absolute best to blur or avoid showing the personalized names printed on the backs of jerseys, we cannot completely guarantee a jersey name won't occasionally slip into the background of an action shot. By agreeing to these terms, you consent to the possible, unintentional use of a jersey name in our promotional content.

Opting Out:

If you prefer that we do not photograph or video your child, no problem at all! Just email us prior to your first class, and we will make every reasonable effort to ensure they remain off-camera (though please understand we cannot always guarantee complete exclusion from wide, full-field group images).

Acknowledgment & Electronic Signature:

I have read this Media Release, fully understand its terms, and agree to grant the permissions stated above. I acknowledge that by toggling "YES" or checking the acceptance box during account completion and checkout, I am providing my electronic signature and agreeing to be bound by these terms.
